Female pattern hair loss (FPHL) is hormonally driven, just like the male version — but the treatment toolbox is different. Women generally can't take finasteride (teratogenicity risk, inconsistent results in female studies), which leaves a gap in the pharmacological options. Elevated potassium can cause cardiac arrhythmias. This is why your doctor checks potassium levels via blood work, typically at baseline, 4–6 weeks after starting, and periodically thereafter. In healthy young women with normal kidney function, clinically significant hyperkalemia is uncommon at hair-loss doses, but monitoring is non-negotiable.
Menstrual Irregularity
Common, especially in the first few months. Spironolactone can cause breakthrough bleeding, irregular cycles, or lighter periods. For many women, this stabilizes after 3–6 months. For those on hormonal contraception, the effect is often minimal. If menstrual changes are significant and persistent, dose adjustment may help.
Breast Tenderness
The anti-androgen effect can cause breast tenderness or mild enlargement. Usually dose-dependent and most noticeable in the first months. It's the same mechanism that makes spironolactone useful in some PCOS contexts — and the same reason it causes gynecomastia in men and is therefore not used for male hair loss.
Other Effects
Lightheadedness (blood pressure reduction — it's still a blood pressure medication), increased thirst and urination (diuretic effect), fatigue, and occasional GI discomfort. Most are dose-dependent and manageable.
Pregnancy contraindication
Spironolactone is Category X in pregnancy — it can cause feminization of a male fetus. Reliable contraception is mandatory for women of reproductive age taking spironolactone. Spironolactone and Hair Transplant Surgery
For women considering a hair transplant, spironolactone plays a complementary role:
Before surgery: Stabilizing hair loss with spironolactone (and/or minoxidil) before a transplant is often recommended. This ensures the pattern is stable, gives the surgeon a clear picture of what's androgenetic versus what might be telogen effluvium, and protects native hair around the transplanted grafts post-surgery.
After surgery: Continuing spironolactone after transplantation protects the non-transplanted native hair from further miniaturization. Transplanted follicles (taken from androgen-resistant donor areas) don't need the medication to survive, but your remaining native hair does. Stopping spironolactone after a transplant risks watching your non-transplanted hair thin away while the transplanted hair stays — creating an unnatural pattern over time.
Surgery day: Spironolactone doesn't need to be stopped for FUE surgery. Its blood-thinning effect is negligible at hair-loss doses. Continue your normal dose unless your surgeon specifically instructs otherwise.
Who Is Spironolactone Right For?
Spironolactone works best for women with androgenetic alopecia — the hormonal, genetically-driven pattern hair loss that causes diffuse thinning, particularly at the part line and crown (the Ludwig pattern). It's also useful for women with hair loss associated with PCOS, hyperandrogenism, or hormonal acne (the anti-androgen effect addresses all three).
It's not the right treatment for hair loss caused by thyroid disorders, iron deficiency, autoimmune alopecia, or stress-related telogen effluvium. This is why a thorough diagnostic workup before starting any medication is essential — you need to know what you're treating before you pick the treatment.
