But here is the truth: shock loss is normal, expected, and not a sign that your transplant failed.
Shock loss — shedding of transplanted (and sometimes surrounding native) hairs 2 to 6 weeks after surgery — is a normal part of the hair growth cycle. The follicles are intact and alive beneath the skin. New growth typically begins at months 3 to 4. Nearly all transplanted hairs shed before they regrow.
What Happens Biologically
When a hair follicle is extracted from the donor area and transplanted into the recipient area, it undergoes significant trauma. The follicle's blood supply is severed during extraction and must re-establish vascular connections in its new location. During this transition, the follicle enters the telogen (resting) phase and sheds its hair shaft. This is not the follicle dying — it is the follicle conserving energy while it heals and re-vascularizes.

Shock Loss of Native Hair
Some patients also experience temporary shedding of their existing native hair around the transplant area. While more distressing than transplanted-hair shedding (because it temporarily makes the area look thinner than before surgery), native hair shock loss is also temporary. Most native hairs that shed from surgical trauma regrow within 3 to 6 months.
When Shock Loss Is NOT Normal
If shedding continues beyond month 3 or if you see no new growth by month 5, contact your surgeon. Factors that can cause poor graft survival include excessive graft handling, prolonged time outside the body (ischemia time > 4 hours), surgical trauma from inexperienced extraction, or untreated underlying conditions (thyroid, iron deficiency). These are surgeon/clinic quality issues — not inevitable outcomes.
Maintaining your finasteride or minoxidil regimen (if prescribed) is important during this period to protect native hair. Some surgeons also recommend PRP (platelet-rich plasma) sessions at months 1 and 3 to support graft survival, though evidence is mixed.
