If you're losing hair and searching for alternatives to pharmaceutical treatment, you've encountered the herbal remedy landscape — a mix of traditional medicines, supplement-company marketing, and the occasional ingredient with legitimate research behind it. The challenge is telling them apart.
This isn't a dismissal of natural approaches. Some of the compounds below have genuine mechanisms of action, plausible biology, and at least preliminary clinical data. Others have centuries of traditional use but minimal modern evidence. And at least one is actively dangerous. The goal is to help you distinguish between these categories before spending money or, worse, taking risks with your health.
The Evidence Tiers
We've organized the most commonly marketed herbal hair loss treatments by what the published research actually supports — not what supplement labels claim.

The Ones Worth Knowing About
Saw Palmetto: The Natural Finasteride (Sort Of)
Saw palmetto (Serenoa repens) is the most-studied herbal remedy for androgenetic alopecia, and the one with the most plausible mechanism. Like finasteride, it inhibits the enzyme 5-alpha reductase, which converts testosterone to the follicle-damaging DHT. Unlike finasteride, it's a weak inhibitor — think of it as finasteride's much gentler cousin.
Several small randomized trials have compared saw palmetto to placebo and found modest improvements in hair count and density. One widely cited study used 320mg daily of a standardized extract and found roughly 60% of participants showed improvement after two years — a respectable number, but the study was small and the improvements were substantially less dramatic than what finasteride trials demonstrate.
The honest comparison
Finasteride reduces scalp DHT by roughly 60–70%. Saw palmetto's DHT reduction is estimated at around 30% in the limited studies that measured it. If you're looking for a meaningful DHT blocker and can tolerate finasteride, saw palmetto is not equivalent. If you can't or won't take finasteride — and you understand you're accepting a weaker effect — saw palmetto is a reasonable option to discuss with your doctor.
Rosemary Oil: One Good Study, Warranted Attention
A 2015 randomized trial compared topical rosemary oil to 2% minoxidil over six months in men with androgenetic alopecia. The results were surprisingly competitive — rosemary oil matched 2% minoxidil in hair count, with significantly less scalp itching.
This is genuinely interesting, but context matters. The study compared rosemary to 2% minoxidil (the weaker over-the-counter concentration), not the 5% formulation most dermatologists recommend. The sample size was small. And no study has compared rosemary oil to modern treatments like low-dose oral minoxidil or combination therapy.
As a low-risk topical addition to a broader treatment plan, rosemary oil is reasonable. As a sole treatment for progressive male-pattern hair loss, the evidence doesn't support that level of confidence. The practical challenge is also real: massaging diluted essential oil into your scalp daily is messy and imprecise compared to taking a pill or applying a measured foam dose.
Pumpkin Seed Oil: Promising but Thin
A Korean randomized trial published in 2014 found that men taking 400mg of pumpkin seed oil daily for 24 weeks had a 40% increase in hair count versus 10% in the placebo group. The mechanism appears related to mild 5-alpha reductase inhibition, similar to saw palmetto.
The result is encouraging, but it's one study from one research group, and the supplement industry cited it heavily without waiting for replication. If further trials confirm the finding, pumpkin seed oil could join saw palmetto in the "weak but real effect" category. For now, it's a "watch this space" ingredient — not a treatment you can rely on for progressive loss.
The Dangerous One: Polygonum Multiflorum
Liver safety warning
Polygonum multiflorum (also marketed as He Shou Wu, fo-ti, or Chinese knotweed) has documented hepatotoxicity. Multiple case reports describe liver injury — including cases of acute liver failure requiring transplantation — associated with its use. Regulatory authorities in the UK, Australia, and several other countries have issued safety alerts. The hair loss evidence is weak, and the liver risk is not theoretical.
He Shou Wu occupies a unique position in the herbal hair loss market: it has a compelling traditional narrative (the name translates roughly to "Mr. He's black hair"), widespread availability in supplement form, and active marketing as a "natural" solution for graying and thinning hair.
What it doesn't have is strong efficacy evidence or a clean safety profile. The hepatotoxic compounds (anthraquinones including emodin) are present in both raw and processed forms, though processing methods affect concentration. The problem is that supplement consumers have no way to verify processing quality, anthraquinone content, or individual susceptibility to liver toxicity.
This is one of the few herbal remedies where the risk-benefit calculus is clear: the evidence for hair benefits is weak, and the evidence for liver harm is established. There are better options with better safety profiles.
What About Combination "Hair Growth" Supplements?
Most commercial hair growth supplements combine several of the ingredients above with vitamins (biotin, zinc, iron, vitamin D) and proprietary blends. A few observations:
Nutritional deficiencies do cause hair loss — and correcting them restores growth. If you're deficient in iron, zinc, vitamin D, or biotin, supplementation genuinely helps. The problem is that most people buying hair supplements aren't deficient. A $50/month supplement stack addressing deficiencies you don't have won't do anything a balanced diet doesn't already do.
The "proprietary blend" label often hides underdosed ingredients. If a supplement contains saw palmetto but the label says "proprietary blend 500mg" that includes six other ingredients, you're probably getting far less than the 320mg dose studied in trials. The blend exists so the company can list impressive ingredients without using effective doses of any of them.
Industry-funded studies dominate the supplement hair-growth evidence base. This doesn't automatically invalidate results, but it does mean the studies that don't show benefit tend not to get published. The publication bias is real and worth factoring into your assessment.
The biotin warning your supplement doesn't mention
High-dose biotin (often 5,000–10,000 mcg in hair supplements) can interfere with laboratory blood tests, including thyroid panels, cardiac troponin (used to diagnose heart attacks), and hormone assays. Several cases of misdiagnosis have been attributed to biotin interference. If you're taking a hair supplement containing biotin, tell your doctor and stop taking it 72 hours before any blood work. Where Herbal Remedies Fit in a Real Treatment Plan
Here's the practical framing, stripped of both supplement-industry hype and pharmaceutical dismissiveness:
If your hair loss is early and mild (Norwood 2, slight thinning), and you prefer to start with low-intervention approaches, a combination of topical rosemary oil, saw palmetto, and a good blood panel to check for nutritional deficiencies is not unreasonable. You should know you're accepting a weaker effect than medication, and you should set a clear timeline (6–12 months) to evaluate whether it's working before your loss progresses further.
If your hair loss is moderate to advanced (Norwood 3+), or progressing noticeably, herbal remedies alone are very unlikely to be sufficient. This is where finasteride, minoxidil, and eventually surgical restoration enter the picture — and where delaying proven treatment in favor of weaker alternatives has real consequences. Hair follicles that miniaturize beyond a certain point don't come back, regardless of what you apply to them later.
Herbal remedies can complement proven treatments. They shouldn't replace them when the evidence gap is this wide.
