FUE: The Versatile Standard

Follicular Unit Extraction is the foundation technique — both Sapphire and DHI are variations of FUE. In standard FUE, individual follicular units (containing 1–4 hairs each) are extracted from the donor area using a micro-punch tool. The surgeon then creates tiny channels (slits) in the recipient area with a steel blade, and grafts are placed into these channels with forceps.

FUE is the most widely available technique globally, produces no linear scar (unlike the older strip/FUT method), and is suitable for the widest range of graft counts. It is the best-value option for most patients.

Sapphire FUE: Faster Healing

Sapphire FUE is identical to standard FUE in extraction and implantation — the difference is the blade used to create recipient channels. Sapphire crystal blades are smoother and harder than steel, creating cleaner, smaller V-shaped incisions (vs U-shaped steel incisions). The result is less tissue damage, faster healing, reduced scabbing, and potentially denser packing because the smaller channels allow grafts to be placed closer together.

The premium over standard FUE is typically $500–$1,000 in Colombia. For most patients, this is worth it — the faster healing is a real advantage, especially for those returning to work quickly.

DHI: Maximum Precision

Direct Hair Implantation uses a Choi implanter pen — a hollow needle that holds a single graft and simultaneously creates the channel and deposits the graft in one motion. This eliminates the separate channel-creation step, reducing the time grafts spend outside the body (which may improve survival rates) and allowing the surgeon to control the exact angle, depth, and direction of each implanted hair.

DHI excels at hairline design (where angle and direction are critical for natural appearance), dense packing in specific areas, and unshaved procedures (the Choi pen can implant between existing hairs without shaving the recipient area).

The trade-off: DHI is slower than FUE for large sessions. A 3,000-graft FUE takes 6–8 hours; the same count via DHI may take 8–10 hours. For very large sessions (4,000+), most surgeons prefer FUE or Sapphire FUE.

Which Should You Choose?

FUE if you need a large session (3,000+ grafts), want the best value, and are comfortable with standard healing time. Sapphire FUE if you want faster healing and slightly denser results for a modest premium. DHI if your priority is hairline precision, you want an unshaved procedure, or you need maximum density in a specific area.
