Yet most hair transplant marketing barely mentions diabetes, and most endocrinologists don't think to discuss cosmetic surgery candidacy. The result is an information gap — patients either assume they can't get a transplant at all, or don't realize their blood sugar control matters for the outcome.
The short answer: well-controlled diabetes is generally not a contraindication to hair transplant surgery. The longer answer involves understanding why control matters, what "controlled" means in this context, and what your surgeon should be asking — and what you should be asking them.
Why Diabetes Matters for Graft Survival
A hair transplant is a minor surgical procedure, but each transplanted graft is a tiny wound that needs to heal and establish a blood supply in its new location. Diabetes affects this process at several levels:
Microvascular Circulation
Chronically elevated blood sugar damages small blood vessels — the same ones that supply the scalp. Transplanted grafts depend on neovascularization (new blood vessel formation) in the first 48–72 hours to survive. Impaired microcirculation means slower and potentially less complete revascularization, which can reduce graft take rates.
Wound Healing
Diabetes slows wound healing through multiple mechanisms: reduced collagen synthesis, impaired immune cell function, and decreased growth factor activity. For a hair transplant, this means donor-area punctures and recipient sites both heal more slowly. In most well-controlled patients, this manifests as a slightly extended healing timeline rather than a clinical problem. In poorly controlled patients, the risk of complications — delayed healing, infection, poor scarring — rises meaningfully.
Infection Risk
Elevated blood glucose suppresses immune function. A hair transplant involves hundreds to thousands of tiny skin openings, each one a potential entry point for bacteria. The infection rate in hair transplants is already low (well under 1% in experienced clinics), but uncontrolled diabetes shifts that baseline upward.
The A1c question
A1c (glycated hemoglobin) reflects your average blood sugar over the previous 2–3 months. Most hair transplant surgeons use it as the primary screening tool for diabetic candidates. A recent A1c result — not one from six months ago — should be part of your pre-surgical workup.
The Candidacy Framework
There's no universal A1c cutoff published in transplant surgery guidelines, but the consensus among experienced surgeons falls into predictable tiers:

Your surgeon should be looking at the full clinical picture, not just one number. If they ask only about your A1c and nothing else, that's worth noting.
Type 1 vs. Type 2: Different Considerations
Both types of diabetes affect candidacy through the same mechanisms (blood sugar → vascular/healing/immune effects), but the practical considerations differ:
Type 2 Diabetes
The majority of diabetic hair transplant candidates have Type 2. Most are managed with oral medications (metformin, SGLT2 inhibitors, GLP-1 agonists) and/or injectable insulin. Key considerations include the duration of the disease (longer duration = more vascular changes), the presence of complications, and current medication regimen. Metformin is generally continued through surgery. GLP-1 medications may need to be paused before surgery due to gastroparesis risk during anesthesia — discuss this with both your endocrinologist and surgeon.
Type 1 Diabetes
Type 1 patients face the same candidacy framework, with the added consideration of insulin management on surgery day. Hair transplants under local anesthesia don't require fasting, which simplifies insulin dosing compared to general-anesthesia procedures. Your surgeon should coordinate with your endocrinologist on whether to adjust your basal or bolus insulin for the procedure day, and should have glucose monitoring capability during the session.
Pre-Surgery Checklist for Diabetic Patients
If you have diabetes and are considering a hair transplant — whether in Colombia or elsewhere — these items should be in place before you book:
- Recent A1c (within 30 days of surgery). Not last year's number. Not a fasting glucose from this morning. Your surgeon needs the A1c trend, ideally with a couple of data points showing sustained control.
- Complete medication list shared with your surgeon. Include insulin types and doses, oral medications, supplements, and any blood thinners or anti-inflammatory drugs. Some medications affect bleeding or healing and may need adjustment.
- Endocrinologist clearance letter (recommended for A1c 7.0–8.0% or any diabetes with complications). This documents that your doctor considers you stable enough for an elective procedure and outlines any perioperative medication adjustments.
- Recent kidney function and complete blood panel. Diabetes-related kidney impairment affects healing and medication clearance. Your surgeon should have a current metabolic panel.
- Realistic conversation about graft survival expectations. In well-controlled diabetes, published graft survival rates appear comparable to non-diabetic patients. In less-than-ideal control, your surgeon should be transparent about potentially lower take rates rather than promising the same outcomes regardless.
Surgery Day and Recovery Differences
For a diabetic patient with a cleared A1c, the procedure itself is identical — FUE or DHI under local anesthesia, same graft counts, same technique. The differences are practical:
Surgery day: Eat normally. Maintain your regular medication schedule unless specifically instructed otherwise by your endocrinologist. Bring your glucose monitor, insulin supplies (for insulin-dependent patients), and fast-acting glucose in case of hypoglycemia. Your clinic should be prepared to check blood glucose during a long session.
Recovery: Follow standard aftercare protocols, with extra attention to keeping recipient sites clean and monitoring for any signs of infection (unusual redness, swelling, warmth, discharge) — which you should report immediately rather than waiting. Healing may take slightly longer, particularly in the donor area. Exercise return timelines are the same, but monitor blood sugar closely during the initial recovery period, as reduced activity and stress can affect glucose levels.
Medications: Post-operative antibiotics are standard in most clinics and especially important for diabetic patients. Finasteride and minoxidil — the medications used to protect non-transplanted hair — don't interact with diabetes medications and can be started on the normal post-transplant schedule.
The good news
The limited published data on hair transplant outcomes in diabetic patients is reassuring. When A1c is below 7% and no significant complications are present, graft survival and aesthetic outcomes appear comparable to non-diabetic patients. The key variable isn't whether you have diabetes — it's how well it's managed.
Red Flags: When a Surgeon Isn't Asking Enough
Your diabetes should be part of the consultation conversation, not an afterthought. Be concerned if a surgeon:
- Doesn't ask about your A1c or request recent lab work
- Clears you based on a fasting glucose alone (one morning's number says almost nothing about your 3-month trend)
- Dismisses diabetes as "not a factor" without evaluating your specific situation
- Doesn't ask about diabetes complications (neuropathy, retinopathy, nephropathy)
- Can't describe what on-site glucose monitoring or emergency protocols look like for diabetic patients
